Introduction

Sei Gelugur Village, located in Pancur Batu District, Deli Serdang Regency, is a rural area inhabited by around 4,266 people, consisting of 1,284 families. The village is home to 345 children under the age of five, who are vulnerable to various health issues, including diarrhea. According to data from the Sukaraya Puskesmas in 2015, diarrhea is the third most common disease in the village, with 513 cases reported. One of the factors contributing to the high incidence of diarrhea in the village is the presence of chicken farms located close to residential areas. The non-sanitary waste treatment on the farm can create an environment that supports the spread of disease, including diarrhea.

The Importance of Sanitation and Environmental Health

Sanitation and environmental health are crucial factors in maintaining public health, especially in rural areas like Sei Gelugur Village. Research Purpose and Methodology

This study aims to analyze the physical condition of the chicken farm cage, the density of flies in the homes of residents, and the incidence of diarrhea in children under five in Sei Gelugur Village in 2016. The method used in this study is a descriptive survey, with a total sampling as a sampling technique, which involved 64 mothers as respondents.

Research Results

The results of this study showed that the physical condition of the chicken farm cage in Sei Gelugur Village did not meet health standards, as the distance between the cage and the residential area was less than 10 meters. However, the processing of livestock manure in general was in accordance with Minister of Agriculture Regulation No. 50/OT.140/10/2006 concerning Poultry Maintenance Guidelines in Settlements.

In measuring the density of flies, which was carried out using a fly grill tool, it was found that 54.7% of households were in the category of moderate flies. This shows the potential for higher disease spread, considering that flies are known to carry pathogens that cause diarrhea.

In terms of child health, data shows that in the past month, 18.8% of toddlers have experienced diarrhea. This figure is quite significant and reflects the need for more attention in terms of sanitation and environmental health.

Recommendations

From the results of this analysis, an important recommendation for relevant agencies such as Puskesmas and the Health Office is to increase counseling on the importance of basic sanitation, environmental cleanliness, and clean and healthy life behavior to the people of Sei Gelugur Village.

For mothers who have toddlers, it is very important to increase awareness about how to maintain cleanliness in the household and the surrounding environment, in order to reduce the risk of diarrhea in children. Education on better livestock waste management also needs to be conveyed so that the negative impacts on public health can be minimized.

Conclusion

In conclusion, this study highlights the importance of sanitation and environmental health in maintaining public health, especially in rural areas like Sei Gelugur Village. The presence of chicken farms in close proximity to residential areas can lead to the spread of disease, including diarrhea. Therefore, it is essential to increase awareness and education on basic sanitation, environmental cleanliness, and clean and healthy life behavior to the people of Sei Gelugur Village. By doing so, it is expected that public health, especially children under five, can be maintained well, and the incidence of diarrhea can decrease significantly in the future.

Limitations of the Study

This study has several limitations, including the small sample size and the limited scope of the study. Future studies can build on this study by increasing the sample size and scope to include more variables and factors that contribute to the incidence of diarrhea in children under five.
