Introduction

Diabetes mellitus is a chronic disease that has become a significant public health concern worldwide. In Indonesia, the number of diabetes sufferers is expected to increase from 8.4 million in 2000 to around 21.3 million in 2030. The conventional treatment of diabetes mellitus involves insulin injections and the use of oral antidiabetic drugs, which can cause side effects and require high costs. Therefore, there is a growing interest in using natural ingredients as an alternative treatment option.

Natural Ingredients for Diabetes Treatment

One of the natural ingredients that has been studied for its potential antihyperglycemic activity is the leaves of the inai (Lawsonia inemis L.). The inai leaf has been used in traditional medicine for various purposes, including the treatment of diabetes. The study aims to evaluate the antihyperglycemic activity of the ethyl acetate fraction of ethanol extract of inai leaves, as well as observing the histological picture of the pancreas in mice induced by streptozotocin.

Methodology

The study involved the use of mice as the experimental model. The mice were induced with 55 mg/kg streptozotocin/kg body weight intraperitoneal. The mice were then divided into seven groups, where groups one to six were mice that were first induced by streptozotocin, while group seven was given an active fraction at a dose of 600 mg/kg body weight before the induction of streptozotocin as a prevention effort. The measurement of blood glucose levels was done using a glucotest on the third, sixth, ninth, twelfth, and fifteenth day after treatment. Results

The results of phytochemical analysis show that the ethyl acetate fraction of ethanol extract of inai leaves contains bioactive compounds such as flavonoids, alkaloids, and tannins. This study shows that the ethyl acetate fraction of inai leaf extract can significantly reduce blood glucose levels in diabetes mice. Statistical analysis shows that the average decrease in blood glucose levels in groups that receive ethyl acetate fractions doses of 400, 600, and 800 mg/kg body weight significantly from negative control groups with a P value of 0.003 each; 0.000; and 0.001, all of which are below 5%. In addition, the histological picture of the pancreas mice shows that the area of Langerhans Island in groups given by ethyl acetate fractions is significantly different from the negative control group with a significant value of 0.003; 0.000; and 0.000.

Discussion

The results of this study concluded that the ethyl acetate fraction of the ethanol extract of the inai leaves not only has an antihyperglycemic effect, but also has the potential to prevent oxidative damage to the pancreas Langerhans Island due to the induction of streptozotocin. This finding opens opportunities for the development of herbal-based therapy that is safer and more affordable in managing diabetes mellitus. The use of natural ingredients such as inai leaves shows new expectations in efforts to prevent and treatment of diabetes, and further research is needed to explore other potentials of this material.
