Introduction

The early breastfeeding initiation program (IMD) is a crucial initiative aimed at improving the health of mothers and babies. Padangsidimpuan City Regional General Hospital (RSUD) has been running an IMD program, but the data obtained in 2014 and 2015 showed a decline in the number of normal deliveries that implemented IMD. This study aims to analyze the implementation of the IMD program in Padangsidimpuan City Hospital using qualitative research methods, involving ten informants consisting of five main informants and five triangulation informants.

Background of the Study

The IMD program is a vital component of maternal and child health care. Breastfeeding has numerous benefits for both mothers and babies, including improved infant growth and development, reduced risk of infections, and enhanced mother-infant bonding. However, the implementation of IMD in Padangsidimpuan City Hospital has faced several challenges. The data obtained in 2014 and 2015 showed that only 14.24% and 9.74% of normal deliveries implemented IMD, respectively. This decline in IMD implementation is a concern, as it may have a negative impact on the health of mothers and babies.

Methodology

This study employed a qualitative research design, using descriptive methods to analyze the implementation of the IMD program in Padangsidimpuan City Hospital. The study involved ten informants, consisting of five main informants and five triangulation informants. The main informants included the Hospital Director, the head of the midwifery room, and three midwives who had experience in implementing IMD. The triangulation informants included two midwives who had not implemented IMD, a nurse who had experience in caring for mothers and babies, and a health worker who had knowledge of the IMD program.

Results

The results of this study showed that the scope of infants who received IMD tended to decrease. One of the factors that influenced this decline was the lack of good communication processes between hospitals and midwives in the socialization of IMD. In addition, there was no written work guideline from the hospital that could be a reference for health workers in implementing IMD. The study also found that the quality of midwives was good, with an average education of D3 Midwifery and adequate training. However, there were still shortcomings in facilities such as blankets and baby hats that supported the implementation of IMD.

Discussion

The results of this study highlighted several factors that affected the implementation of the IMD program in Padangsidimpuan City Hospital. The lack of good communication processes between hospitals and midwives was a significant factor, as it hindered the socialization of IMD. The absence of written work guidelines from the hospital also contributed to the decline in IMD implementation. The study also found that the commitment of all parties, including the government and health workers, was not 100% due to the absence of a clear Standard Operating Procedure (SOP) as a guide to the midwife's work in implementing IMD.
