The Level of Knowledge and Compliance with Taking Medication in Patients with Diabetes Mellitus Type 2: A Comparative Study between Susanna Wesley Methodist Hospital and Putri Hijau Hospital Medan City

Introduction

Diabetes mellitus type 2 is a chronic disease characterized by hyperglycemia and disorders in carbohydrate, fat, and protein metabolism. The management of this disease requires good knowledge about the disease and compliance in treatment to prevent further complications. This study aims to identify the level of knowledge and compliance of patients in taking drugs, as well as analyzing the influence of patient characteristics on their knowledge and compliance. In addition, this research also explores the relationship between the level of knowledge and patient compliance.

Background

Diabetes mellitus type 2 is a major public health concern worldwide, affecting millions of people. The disease is characterized by insulin resistance and impaired insulin secretion, leading to hyperglycemia and various complications such as cardiovascular disease, kidney disease, and nerve damage. Good knowledge about the disease and compliance in treatment are essential to manage these conditions and prevent further complications.

Methodology

This study was conducted using a descriptive method with a cross-sectional approach. The research location was the Susanna Wesley Methodist Hospital in March to June 2022 and the Hijau Putri Hospital in Medan in July to September 2022. A total of 61 respondents were selected for the study. The DKQ-24 questionnaire was used to assess knowledge, while the Mars-5 questionnaire was used to assess compliance. Data analysis was carried out using SPSS version 22 with univariate and bivariate analysis tests.

Results

The results of the study showed that of 61 respondents, 26.2% had high levels of knowledge, 70.5% moderate, and 3.3% were low. Meanwhile, for the level of compliance, 44.3% were in the high category and 55.7% in the medium category. The chi-square test results showed that only the level of education affects the level of knowledge with a value of 0.02, while age affects the level of compliance with a value of 0.03. The Spearman RHO test indicated a significant moderate correlation between the level of knowledge and the level of compliance (r = 0.290).

Discussion

The results of the study showed that the level of patient knowledge is in the medium category, as well as their level of compliance. There is a significant relationship between the level of knowledge and compliance with taking medication in patients with type 2 diabetes mellitus. Good knowledge is very important to increase patient compliance with the treatment they live. The study also found that the level of education affects the level of knowledge, while age affects the level of compliance.

Conclusion

In conclusion, the level of patient knowledge and compliance with taking medication in patients with type 2 diabetes mellitus is a significant concern. The study highlights the importance of good knowledge about the disease and compliance in treatment to manage these conditions and prevent further complications. The hospital can design an educational program that is more effective for patients, so that their quality of life can be improved. Therefore, efforts to increase patient knowledge through health education need to be carried out, including explaining the importance of compliance in treatment to manage type 2 diabetes mellitus optimally.

Limitations

This study has several limitations. Firstly, the study was conducted in two hospitals in Medan City, which may not be representative of the general population. Secondly, the study used a cross-sectional approach, which may not capture the dynamic nature of patient knowledge and compliance. Finally, the study relied on self-reported data, which may be subject to bias.
