Introduction

The temporomandibular joint (TMJ) plays a crucial role in the stomatognathic system, which includes teeth, muscles, maxillary and mandibular bones, nerves, blood vessels, periodontal tissue, and joints themselves. This joint enables various complex movements, essential for mastication, manners, pronunciation, and balance of the oral cavity. However, interference with one component of the stomatognathic system can disrupt the function of other components, resulting in an imbalance. One common issue encountered is temporomandibular joint disorder (TMJ), which can be acute or chronic, affecting the head, jaw, face, and neck area.

Causes and Signs of Temporomandibular Joint Disorders

TMJ disorders have multifactorial causes, which can be categorized into three groups: initiation factors, predisposing factors, and speeding factors. Initiation factors include trauma, excessive chewing burden, and bad habits, while predisposing factors include anatomical, systemic, psychological, and malocclusion aspects. Speeding factors are related to psychosocial conditions and hormone metabolism. The signs and symptoms of TMJ disorders vary greatly, including headache, ear pain, joint pain, muscle pain, mandibular deviation, limitations of mouth opening, and joint sounds.

Methodology and Results

This study aimed to analyze the distribution of signs and symptoms of TMJ disorders and examine the relationship between malocclusion and bad habits with these symptoms in students of the Faculty of Dentistry (FKG) of the University of North Sumatra (USU). A descriptive analytic method with a cross-sectional approach was used, involving 112 people as a sample. Data were obtained through interviews and clinical examinations using the Research Diagnostic Criteria (RDC) Procedure Anamnestic Index and Clinical Examination.

The results showed that of the 65 people (58%) who had malocclusion, there were 39 people (60%) who had headache, 50 people (76.9%) had joint pain, 30 people (46.1%) had muscle pain, 50 people (76.9%) experienced mandibular deviation, 49 people (75.4%) experienced limited mouth opening, and 55 people (84.6%) had joint sounds. In addition, based on bad habits, it was found that 55 people (78.6%) had headache, 38 people (54.3%) had joint pain, 39 people (55.8%) had muscle pain, 55 people (78.6%) experienced mandibular deviation, 49 people (70%) experienced limited opening, and 43 people (61.5%) experienced joint sounds.

Analysis and Conclusion

From the results of the analysis, there is a significant relationship between malocclusion and signs of symptoms of TMJ disorders, with each p-value for joint pain (p = 0.001), muscle pain (p = 0.014), mandibular deviation (p = 0.008), limited opening (p = 0.001), and joint sound (p = 0.002). However, there is no significant relationship between malocclusion and headache (p = 0.245). Likewise, there is a significant relationship between bad habits and signs of TMJ disorders, including headache (p = 0.001), muscle pain (p = 0.001), mandible deviation (p = 0.001), and limited opening (p = 0.002). However, bad habits do not have a significant effect on joint pain (p = 0.354) and joint sounds (p = 0.213).

Implications and Recommendations

This study provides an important insight into the factors that contribute to temporomandibular joint disorders, especially among USU FKG students. By understanding the relationship between malocclusion and bad habits of the symptoms of TMJ disorders, it is hoped that preventive measures and more appropriate treatment can be taken. Increasing awareness about the importance of maintaining dental health and good habits in daily life is very necessary to prevent this disorder.

Further studies are needed to investigate the relationship between malocclusion and bad habits with TMJ disorders in other populations, such as children and adults. Additionally, the development of preventive measures and treatment protocols for TMJ disorders should be based on the findings of this study. By doing so, we can reduce the prevalence of TMJ disorders and improve the quality of life for individuals affected by this condition.

This study had several limitations, including the small sample size and the use of a cross-sectional approach. Future studies should aim to recruit a larger sample size and use a longitudinal approach to investigate the relationship between malocclusion and bad habits with TMJ disorders over time.
