Introduction

Pulmonary tuberculosis (TB) remains one of the infectious diseases that seriously contributes to high morbidity and death in the world. In Indonesia, based on official reports from WHO, the country ranks second with the highest TB burden. The Indonesian government has implemented a pulmonary TB eradication program through TB treatment using DOTS (Directly Observed Treatment Short-Course) strategies since 1995. This strategy is designed to break the chain of disease transmission. Glugur City Health Center is located in Medan and is one of 39 Puskesmas in Medan which records healing rates of 33.33%. This figure shows that this puskesmas has not yet reached the set national target, which is a minimum of 85%.

Background of the Study

Pulmonary TB is a major public health problem in Indonesia, and the government has implemented various strategies to control and eliminate the disease. The DOTS strategy is a key component of the national TB control program, which aims to provide effective and efficient treatment to TB patients. However, the implementation of the DOTS strategy at the Glugur City Health Center has been suboptimal, with a low healing rate of 33.33%. This study aims to analyze the implementation of the Lung TB Management Program with DOTS strategy at the Glugur Puskesmas Medan City in 2018.

Methodology

This study used a qualitative approach to analyze the implementation of the Lung TB Management Program with DOTS strategy at the Glugur Puskesmas Medan City in 2018. The data collection methods used were in-depth interviews and direct observations. The informants in this study included health problems management staff from the Medan City Health Office, the Head of the Glugur City Health Center, Lung TB officers at the Puskesmas, TB patients, and the Supervisor of Drugs (PMO). The data analysis followed the procedures proposed by Miles and Huberman.

Results

The results of this study showed that the implementation of the pulmonary TB control program with DOTS strategies at the Glugur City Health Center was not optimal. Some of the factors that cause the lack of maximum implementation of this program include the quality of pulmonary TB officers who are still below the standard in the case of case discovery. In addition, PMO has not yet carried out its duties properly, which has an impact on the low healing rate that does not reach the national target. The healing rate which only 33.33% shows problems in various aspects of health services.

Discussion

The results of this study indicate that the implementation of the DOTS strategy at the Glugur City Health Center has been suboptimal. The quality of pulmonary TB officers is still below the standard, and PMO has not yet carried out its duties properly. This has resulted in a low healing rate of 33.33%, which is below the national target. The low healing rate is a major concern, as it indicates that the program is not effective in controlling and eliminating TB in the community.

Recommendations

Based on the findings of this study, one of the recommendations proposed is that TB officers intensify the discovery of cases through a more proactive approach. This includes improving communication and cooperation with PMO during the treatment process. In addition, more efforts are needed to socialize and educate the public about pulmonary TB, so that they are more aware of the symptoms and importance of early examination. With these steps, it is expected that the number of case discovery can be increased and the success of TB treatment can be better in accordance with the targets set.

Conclusion

In conclusion, the implementation of the Lung TB Management Program with DOTS strategy at the Glugur Puskesmas Medan City in 2018 was not optimal. The quality of pulmonary TB officers is still below the standard, and PMO has not yet carried out its duties properly. This has resulted in a low healing rate of 33.33%, which is below the national target. The low healing rate is a major concern, as it indicates that the program is not effective in controlling and eliminating TB in the community. Therefore, it is recommended that TB officers intensify the discovery of cases through a more proactive approach and that more efforts are needed to socialize and educate the public about pulmonary TB.

Limitations of the Study

Future research should focus on evaluating the effectiveness of the DOTS strategy in controlling and eliminating TB in the community. Additionally, research should be conducted to identify the factors that contribute to the low healing rate and to develop strategies to improve the quality of pulmonary TB officers and the duties of PMO.
